MoZaic Care uses Athena Health's patient portal so patients can easily access and manage their information, request office appointments and more. It is also optimized for mobile smart phone use to allow patients easy access to their records and results. 

Within the patinet portal, you can:

• View test and lab results

• Send secure messages to your provider and office staff

• 24/7 access to your medical history

• Request appointments

• Request prescriptions and refills

• Make payments online

Once you are an established patient, you can sign-up for the patient portal by clicking the icon above or by requesting our office staff to send you an invite. We encourage all of our patients to sign up for the portal.
